Monosulfiram
25% w/v One-Mix Monosulfiram Solution
Monosulfiram (also called Sulfiram) is an effective ectoparasiticide primarily used for the treatment and prevention of scabies. It is a sulfur-containing compound that acts as a toxin against Sarcoptes scabiei (the scabies mite), killing both mites and their eggs and providing relief from intense itching.
Main Applications
Treatment of Scabies: Effective in eliminating mites from the skin.
Relief from Itching: Helps reduce itching and irritation caused by infestation.
Prophylactic Application: May be used to prevent spread within close contacts or family members.
Directions for Application of Monosulfiram
Dilution: The 25% solution should generally be diluted with 2 parts water (1:2) before use. For children, a 1:3 dilution is recommended or as prescribed by a doctor.
Application Method: After a warm bath, dry the skin thoroughly and apply the lotion over the entire body from neck down. Pay special attention to skin folds, between fingers, and under nails.
Duration: Leave on the skin for 24 hours before washing off. A repeat application after 1 week may be required if infestation persists, only under medical advice.
Precautions a Patient Must Take
1. Antabuse Effect: Avoid alcohol consumption for at least 48 hours after application. Similar to Disulfiram, it may cause flushing, tachycardia, nausea, and redness if alcohol is consumed.
2. External Use Only: Do not allow contact with eyes, mouth, or open wounds.
3. Personal Hygiene: Wash or disinfect clothes, bedding, and towels to prevent re-infestation.
4. Pregnancy & Lactation: Consult a dermatologist before use during pregnancy or breastfeeding.
Potential Side Effects
Some individuals may experience:
Mild skin irritation or redness
A temporary tingling sensation
Erythematous rash (in sensitive individuals)
Discontinue use and seek medical advice if severe irritation, dryness, or allergic reactions (such as hives or swelling) occur.
Frequently Asked Questions (FAQ)
Q: Why do I still itch after applying Monosulfiram?
A: Post-scabetic itch can persist for 2–3 weeks even after mites are eliminated. This is due to an allergic reaction to dead mites and their waste.
Q: Can I use Monosulfiram on my face?
A: Generally not recommended. Scabies rarely affects the face in adults. Consult a dermatologist before applying to sensitive areas like the face or scalp.
Q: Is it safe for young children?
A: Yes, but requires greater dilution (usually 1:3). Always consult a pediatrician before use in infants or young children.
Q: When can I return to work or school?
A: Typically, it is safe after 24 hours of the first full-body treatment.
Q: Can I use Monosulfiram if I am allergic to sulfur?
A: No. Since it is a sulfur-based compound, it should be avoided in individuals with sulfur allergy.
