Imeglimin
Dosage & Administration
One dose, once daily, swallowed whole with a glass of water in the morning, approximately 30 to 60 minutes before eating your first meal of the day.
Dosage
Dosage is to be decided by the attending physician.
Adult Dosing: Typically initiated at 500 mg twice daily as an oral medication, with possible titration to achieve the most appropriate dose.
Possible Side Effects
During clinical studies, gastrointestinal symptoms such as diarrhea, nausea, and vomiting were generally mild, indicating that Imeglimin is well tolerated.
Due to its direct mitochondrial action, very mild reactions may occur as treatment progresses.
Administration Procedure
With Meals: Tablets should be taken with meals to improve tolerance.
With Water: Administer with a full glass of water.
Do Not Alter Tablets: Do not crush, split, or chew the tablets, as this may affect drug efficacy.
Safety & Caution
Renal Function: Since Imeglimin is primarily excreted via the kidneys, monitoring of eGFR (estimated glomerular filtration rate) is essential. Dose adjustments may be required in chronic kidney disease (CKD).
Alcohol: Avoid alcohol, as it may increase the risk of hypoglycemia and lactic acidosis.
Pregnancy/Lactation: Inform your healthcare provider if you are pregnant, planning pregnancy, or breastfeeding before starting treatment.
Potential Side Effects
Most patients tolerate Imeglimin well, but possible effects include:
Gastrointestinal: Nausea, diarrhea, abdominal discomfort (usually temporary).
Neurological: Mild headache, itching, or dizziness (rare).
Hypoglycemia: Rare when used alone, but risk increases when combined with insulin or sulfonylureas.
Frequently Asked Questions (FAQs)
Q: Can Imeglimin be taken with Metformin?

Q: Can it replace insulin?
A: No. It is used for Type 2 Diabetes and is not suitable for Type 1 Diabetes or diabetic ketoacidosis, where insulin is required.
Q: How quickly will my HbA1c improve?
A: Blood glucose improvements may appear within 2–4 weeks, while HbA1c reduction is typically seen over 3 months.
Q: What should I do if I miss a dose?
A: Take it as soon as you remember. If it’s close to the next dose, skip the missed dose. Do not double dose.
Q: Will I lose weight with Imeglimin?
A: Imeglimin is generally weight-neutral. However, improved blood sugar control may lead to minor weight changes in some individuals.
